While preparing other holds, which were empty, for the next cargo, a seaman inadvertently opened the access hatch to No.4 hold and , on noting what appeared to be water in the hold with the concentrates, reported this to the bosun. After informing the bridge as to what had been reported, the bosun went down into the hold to investigate but within a short time, he collapsed, falling onto the cargo. This report deals with the investigation into the death of the bosun.
